A Brief Overview
Assists patients and their families in understanding illness and disabilities. Helps resolve personal, financial, and environmental barriers which interfere with obtaining maximum benefits from care. Assists patients/families in developing and achieving a plan of care to obtain maximum benefits.

What you will do  

  • Provide wide range of psychosocial services to cancer/palliative patients and their families, including regularly assessing the patient and family psychosocial needs.
  • Assures psychosocial assessment of patients/families/caregivers and appropriate services are rendered in a timely manner.
  • Directs patients and families to appropriate community agencies and resources for financial assistance, counseling, rehabilitation, auxiliary care and long term care via written and oral communication.
  • Implements the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving.
  • Initiates home care referrals for HNMC VNS, orders durable medical equipment and arranges ambulance transportation as needed for patients.
  • Provides direct psychosocial services/care to patients/families/caregivers and regularly re-evaluates the patient's and family's psychosocial needs.
  • Documents clinical records of psychosocial services utilization and related activities for quality assurance, program development, and policy and procedure review and revision
  • Assist in Quality Improvement projects specific to the department.
  • Maintains a knowledge of resources available in the community for patients and their families.
  • Adheres to policies and procedures and regulatory compliance requirements and directives. Performs timely patient and family screening and interviews with them to assess and evaluate patient's social, psychological and environmental situations



Education Qualifications  

  • Master's Degree Master's Degree in Social Work Required


Experience Qualifications  

  • 1-3 years Minimum of 2 years social service experience in a health care setting or social service agency.


Knowledge, Skills, and Abilities  

  • Analytical and problem solving along with excellent verbal and written communication skills. Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.).


Licenses and Certifications  

  • A NJ Social Work License Required
