A Brief Overview
The Acute Care Nurse Practitioner is an advanced Practice Nurse who provides services to a defined patient population. Facilitates and coordinates patient care services for patients through the continuum of care that ensures safe quality care in the most cost efficient manner.

What you will do

  • Delivers patient care in accordance with the ANA Scope and Standards of Care and Practice, Patient Bill of Rights and the ANA Code of Ethics for Nurses.
  • Performs daily patient rounds with collaborating physician or physician group
  • Coordinates patient care needs according to plan of care and consults with collaborating physician at least daily or as indicated.
  • Collaborates with all members of multidisciplinary health care teams to plan, manage, or assess patient treatments.
  • Order, perform, or interpret the results of diagnostic tests and screening procedures based on assessment results, interventions, medications, patient responses or treatment changes
  • Develops appropriate treatment plan by orders and interprets diagnostic tests, prescribes pharmacologic agents, and administers medications.
  • Document data related to patients care including assessment findings, results, interventions and treatment
  • Obtains a relevant assessment of clients health status by completing H&P and physical exam. Formulates appropriate diagnosis based on findings
  • Uses evidence based practice, current standards of care and research in practice. Participates in the development of practice protocols.
  • Diagnose acute or chronic conditions that could result in rapid physiological deterioration or life-threatening instability.
  • Discuss illnesses and treatments with patients and family members. Provides patient education and appropriate resource information.
  • Collaborates with Discharge Planning Care Coordinators for all discharge needs to ensure efficacious care delivery, increased patient satisfaction and positive patient outcomes.
  • Works closely with Physician Advisor for Outcomes Management to decrease length of stay and payor denials.
  • Participates on various performance improvement committees including but not limited to Utilization Review Oversight Committee (UROC) and Clinical Documentation Steering Committee (CDSC) in order to utilize data to enhance patient care.
  • Completes assignments in a timely manner, thoroughly and appropriately detailed.
  • Demonstrates and understands compliance with all hospital and departmental policies and procedures. Routinely utilizes policy and procedure manuals and other available literature/reference materials to ensure consistency and accuracy on the delivery of acute care.
  • Performs any related duties as assigned.



Education Qualifications

  • Bachelor's Degree in nursing Required and
  • Master's Degree in Nursing Required


Experience Qualifications

  • 1-3 years Minimum of 2 years of experience as a nurse practitioner. Required


Knowledge, Skills, and Abilities

  • Experienced NP should possess sound physical assessment skills; with the ability to manage acute and chronic illness states, incorporating disease management including but not limited to disease lifestyle modifications, nutritional needs, pharmacotherapy, education, and evaluation of treatment plans for individual patients.


Licenses and Certifications

  • RN - Registered Nurse - State Licensure and/or Compact State Licensure Current NJ RN license Upon Hire Required and
  • APNP - Advanced Practice NP Nurse Practitioner certification. Upon Hire Required
