A Brief Overview

Responsible for coordinating the arrangement of ambulance transportation services for patients being transferred from HNMC to post-acute providers or to the patient's home. Position includes following up with care coordinators, social workers, families and the ambulance dispatch center to assure necessary documentation is accurate and timely service is provided


What you will do

  • Arranges ambulance transportation services for patients being transferred to post-acute setting
  • Collaborates with transitional care staff to arrange timely discharges .
  • Answers incoming phone calls and takes messages as needed.
  • Completes the transfer database
  • Assists care coordinators by print and faxing the charts for appealed Medicare cases to QIO
  • Assists with DRG hold list
  • Completes payment requisitions for ambulance company
  • Works with transportation company to resolve complaints (late pick ups)
  • Monthly chart audits.
  • Assists with other tasks as needed.


Education Qualifications

  • High School Diploma High School Diploma Required
  • or equivalent experience Required


Experience Qualifications

  • 1-3 years 2+ years as a transport coordinator


Knowledge, Skills, and Abilities

  • Basic computer skills preferred.
  • Proficiency in MS Office with expertise in Microsoft Word, PowerPoint and Excel.
  • Problem solving along with excellent verbal and written communication skills.
