A Brief Overview

Will evaluate Physical Therapy needs of patients in their home or other community setting, prepare the rehabilitation care plan based on the assessment, provide Physical Therapy services based on the Rehab Care Plan, reassess patient's response to services and revise the Rehabilitation Care Plan as needed. Treats patients of all ages, all types of disabilities and multiple medical problems and collaborates with all team members to maximize patient outcomes. Job requires extensive standing, walking, lifting, crouching, stooping, reaching, kneeling, stair climbing, writing, electronic use, driving and phone use. Responsibilities include making home visits in various areas during inclement weather and exposure to variable environmental conditions in the home setting care experience preferred.


What you will do

  • Evaluates the patient's level of function and physical therapy needs, determines appropriate long and short term goals and develops the Rehabilitation Care Plan based on patients' needs.
  • Create Plan of Care and obtain orders detailing interventions and goals in collaboration with the patients' medical provider.
  • Complete a Comprehensive Assessment as required by the Conditions of Participation which includes verification of eligibility for Home Care benefit including but not limited to homebound status, assessment of skilled need, medication reconciliation and obtain consent for care.
  • Manages a caseload of patient while administering therapeutic exercises and treatments, providing patient/family education, assessing patient progress towards goals and implementing safe discharge planning.
  • Directs and aids patients in active and passive therapeutic exercise, use of rehabilitation modalities, muscle re-education, gait training and activities of daily living to promote independence in the community.
  • Provided patient/family education in the care and use of assistive and adaptive equipment.
  • Incorporates patient safety in all assessment, instructions and training and recommends Home Modification as identified.
  • Incorporates the physiological and developmental age of patients into the overall plan for the delivery of patient care.
  • Participates in staff meetings and other educational venues.
  • Timely and efficient documentation to meet policy guidelines to ensure documentation accuracy.
  • Able to use electron software system for all patient care documentation.
  • Demonstrates compliance with the organizations dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.
  • Coordinates schedule to comply with visit frequency and plan of care.
  • Performs additional related duties as assigned.


Education Qualifications

  • Bachelor's Degree of Science Degree in Physical Therapy required or
  • Master's Degree of Science Degree in Physical Therapy required


Experience Qualifications

  • Home Care/Community Health Setting Experience required


Knowledge, Skills, and Abilities

  • Knowledge of Home Exercise Programs and Equipment.
  • Knowledge of Home Care insurance reimbursement, State, Federal and Joint Commission Standards for Home Care
  • Capable of frequent lifting and transferring of weight of to 300 pounds.



Licenses and Certifications

  • PT - Physical Therapist New Jersey Licensed upon hire required and
  • BCLS - Basic Life Support American Heart Association upon hire required
  • DL NUMBER - Driver License, Valid and in State Drivers License upon hire required

Pay Range: $42.40/hour - $53.00/hour
